Create a new Form, or open an existing one.

  1. Create two (2) sections
  2. Add two (2) multiple choice questions
  3. Add one (1) short answer question
  4. Add one (1) dropdown question
  5. Change the theme color

Share a link to the form you created. Ensure you check the form settings to uncheck the 'restrict users to your organization' box to allow your form to be viewed by the approving team.
